Once submitted, you will be contacted by one our staff members with further information on your qualification to receive an application. If your family does qualify for assistance, you will receive a separate application that requests more information from you including the bills for which you are requesting assistance. At this time, to qualify for assistance, your child must be treated at one of the three Central Florida Hospitals (AdventHealth for Children, Arnold Palmer Hospital for Children, or Nemours Children's Health) OR live in one of the seven counties we serve (Orange, Osceola, Lake, Volusia, Brevard, Polk, or Seminole). There are exceptions for families traveling to and/or from Central Florida to receive treatment.
