*Shift times throughout this time frame are selected, the entire duration of the day does not need to be completed.

Patient Family Sign Up

In order to participate as a model at the 2024 Spring Soiree, your family must meet all requirements below and fill out the following documentation

Diagnosis

Your child has been diagnosed with Pediatric Cancer

Age Requirement

Your child is between the ages of 6 months old and 18 years old

Status

Your child is in Active Treatment or has been in Remission for no longer than 5 years

Treatment Location

Your child is being treated at one of the three Central Florida Children's Hospitals

Is my child eligible to participate?
